Managing Patient Health Data

You can create and manage a reference to the location of patient health information data using the Patient Health Data template. You can electronically record the details of the patient in reference to the health information data from an external source. You can open the external page using the Access Link field. You can open the Patient Health Data template from the navigation bar that is available for all specialties. The Patient Health Data template includes the Open to Add/Update panel. By default, the Open to Add/Update panel is collapsed. When you expand the Open to Add/Update panel, the panel name appears as Close Add/Update. Create Health Survey Reports

You can enter health survey details of a patient on the Health Survey Information window. Before you begin You must have NextGen® Enterprise Rosetta Interface Messenger to transmit the health survey report to public health agencies. Open NextGen® Enterprise EHR. Select a patient encounter, and open a template.For example, Intake. Select Navigation, and then select the Health Survey Info link.The Health Survey Information template opens. Health Survey Information To indicate the type of visit, select one of the following Episode of care options: Initial Patient Assessment Follow-up Visit In the Major Reason for Visit field, select a reason in the list. For asthma health survey reporting, do the following: In the Asthma problem field, select an asthma problem in the list. Note: The list includes the qualifying asthma problems documented in the Problems module or the Problems List panel. The Condition Control and Severity fields are enabled for documentation. Document Functional Status Assessment for Chronic Conditions

You can document the functional status assessment for chronic conditions. Open NextGen® Enterprise EHR. Select a patient encounter, and open a template. For example, Intake. Select Navigation, and then select the Document Library link. The Documents and Letters template opens. Select the Veterans Rand 12 Item Health Survey (VR-12) link. Print and give the Veterans Rand 12 Item Health Survey (VR-12) questionnaire to the patient for completion. Open the navigation bar, and then select Screening Tools. The Screening Tools template opens. Select the Veterans Rand 12 Item Health Survey (VR-12) link. The Veterans Rand 12 Item Health Survey (VR-12) template opens. Enter the information provided by the patient in the Veterans Rand 12 Item Health Survey (VR-12) questionnaire. The patient's score is filled automatically. To save the information, select OK. Prerequisites for Reporting Asthma Issues

You can report asthma problems to health survey agencies. Before reporting asthma problems to health survey agencies, ensure that you complete the following: Add the asthma related SNOMED codes on the Data Sets Setup practice template using the Asthma Codes data set. Note: For more information on the asthma SNOMED codes configuration, go to NextGen Healthcare Success Community, and download the Data Sets Setup Practice Template User Guide for NextGen® Adaptive Content Engine. Document the asthma codes as the active asthma problems in the Problems module or the Problem List panel on the Histories template. Accessing and Setting up Templates from Navigation Bar

You can access and configure various templates in the Navigation bar. The navigation bar consists of links for quick navigation. You can generate notes, open templates, document a patient's education and health data, manage referral and orders, and so on. Based on the configuration done on the Custom Navigation template, the links appear on the navigation bar. Document Patient's Mental Health Details Using the Brief Assessment Template

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. From the list, select Brief Assessment (BH), and then select OK. Select the program, specialty, and visit type for the patient. Select whether you have discussed confidentiality and obtained patient's consent for the care. To document the patient's past medical history, select the History Summary link. In the Presenting Concerns panel, do one of the following actions in the Written HPI narrative field: Enter the patient's symptoms and presenting issues that prompted a visit to the office. Use the My Phrases link to select the details for the visit. In the Substance Use/Addictive Behavior panel, select the patient's addiction details to substances.
