Record a Risk Assessment

Any assessment of risk data that was entered previously on the Comprehensive Assessment template set carries forward to the panels found here. On the Risk Assessment panel, if the patient is not a danger to self or other, select the option for Patient is not currently a danger to self, others, or property. Selecting this option sets every field in the section as unavailable. The Encounter field fills with the current date. In the Risk field, select the appropriate type of risk from the list. If there is a need to record a previous risk for the client, select the Past option. CAUTION: Always document past risks before a current assessment. If Past is selected, either an Event Date (even if approximate) or Denied should be populated. If there are no past assessments to document, select the option for Current. Select Denied or Admits. If Denied is selected, the documentation options are all disabled in this section except for the date fields and the Add and Clear buttons. Enter the Event

Record Substance Use and Addictive Behavior Histories

On the Substance Use/Addictive Behavior History panel, select the None reported checkbox if the client denies substance use or addictive behavior. Select one or more of the addictions options reported by the client. Use the Other field can be selected to type an historical issue that is not available on the panel in the associated field. Select the client's response for Drinks alcohol. Click the Tobacco Usage link to enter the client's tobacco use. If any of the addiction options apply and a more in-depth evaluation is required, click the Substance Use link to enter more details. The BH Substance Use Assessment template launches. Document Risk Assessment

Open a risk assessment template, and expand the Risk Assessment panel. To document a potential for suicide and homicide risk, select one or more of the following links: Suicidal/Homicidal Risk Safety Risk Columbia Suicidal Severity Rating Scale (C-SSRS) SAFE-T Protocol with C-SSRS (Columbia Risk and Protective Factors) Note: Only one of the C-SSRS template appears based on the configuration in the BH Foundation Configuration template. To open the risk history view, select the Risk Assessment History link. The Risk Assessment History window opens. If the client is not currently and has never been a danger to themselves, others, or property, select the respective checkbox. If selected, the remaining fields are turned off. Document the Patient's Mental Health and Risk Assessment Information

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. From the list, select Comp Assess - BH Hx, and then select OK. To document the patient's medical and surgical details in the Past Medical/Surgical panel, select Add. Note: To document the patient's mental and physical health details, you can select the corresponding links in the Past Medical/Surgical panel. In the Substance Use/Addictive Behavior History panel, document the details about the patient's addiction to substance use. Enter Crisis Treatment Plan

On the BH - Crisis Treatment Plan template, expand the Crisis Treatment Plan panel. Click the Suicide/Homicide Risk link to assess risk level. Crisis Treatment Plan The Safety Risk template opens. Safety Risk Click the Quality button to learn about the Quality Reporting requirements related to this type of risk. Click Save & Close to return to the BH - Crisis Treatment Plan template. Under Problem/Precipitating Events, manually type an explanation in the field. In the Signs of Risk field, enter a description of the signs, symptoms, and circumstances that may cause the documented risk to happen again. Under the Strategies to Reduce Risk section, use the Patient strategies and Staff interventions fields to document specific actions meant to lower the risk. In the Goals Prior to Release from Precautions section, record the target initiatives to be completed by the client before being released from precautions. Document the Patient's Relapse Prevention Plan

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Relapse Prevention Plan (BH), and then select OK. Note: You can open the Relapse Prevention Plan (BH) template from your Packet Navigation or you can open the template from the Templates module. For more information on configuring Packet Navigation, go to NextGen Healthcare Success Community, and download the Configuration Guide for NextGen® Behavioral Health Suite. In the Time Entry Navigation sidebar, select Billing Time Entry, and then enter the start time. Note: You can select the current time or enter the time manually. Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list. You can select <No Program> to create an encounter without a program, and then add the program later. Document the patient's admission date, plan creation date, and plan revision
