Access the BH - Psychopharm Template

To access the BH - Psychopharm template, set the Specialty to Behavioral Health and then select one of the following for the Visit Type: Medication Management with Psychotherapy Medication Management with Psychotherapy & Translator Click the Psychopharm F/U tab. Enter the Prescriber's Evaluation

Expand the Prescriber's Evaluation panel. If there are no modifications, select the No changes reported/observed checkbox. In the Interim Update field, use the My Phrases link or manually type information about the client's progress since the last visit related to symptoms, effectiveness of medications, substance use, changes in family/social history, overall functioning, and any new issues. Enter Document Signatures

Multiple people may be required to sign a treatment plan. Security on the Signatures panel is set up so that only the person who checks a signature line can alter it in any way. The name of the user who is logged on to the computer populates in the name field, along with the date of the signature. After ‘signing’, the signer clicks on the Send Task button in the upper right of this panel. The template is sent to the next person who has to sign off. Electronic signature software can be used to capture the signature for client and/or parent/guardian. To enter signatures on the Signatures panel, place a check mark next to the applicable label(s). Enter any Credentials to further identify a signature as needed. Select N/A to indicate that a signature is not applicable and clear the fields of anything that may have been entered in the same row. Record Plan Details

On the BH - Psychopharm template, navigate to the Plan panel. Document the Diagnosis since the last visit by selecting from the following: No change Yes, CA update required Next to Rationale for changing medications, select Not indicated or enter an explanation in the associated field. Answer No or Yes to: Were Medications delivered today. Next to If Yes, for what duration select one of the following options: Weekly Monthly Other Select No or Yes to indicate if there was a Prescription written. Document if the medication was Called into pharmacy. Next to Lab tests were ordered, select No or Yes. If Yes, enter available Results in the associated field Click the Review Labs link to open the Orders module and to view existing lab orders for this client. On the BH - Individual Counsel/Psych Prog Note template, expand the Program Name panel. Click the Program name field to select the appropriate name. To enter the Start time, do one of the following: Click the clock button to enter the current time in the field. In field, click to manually type the time. Previously entered times can be modified by clicking in the field. It is important to follow the established format: 0:00 AM or PM.
