Patient's Physical Exam

You can examine a patient and document the associated information using the Physical Exam panel that displays a list of exam categories. The links that appear are configurable by specialty or by provider. The Physical Exam panel displays the system name, followed by exam findings, and the details. On Physical Exam, you can use the One Page Exam link in the left pane to open the One Page-Physical Exam template, which enables you to document the complete physical exam details on a single template. You can also document a single system exam. Physical Exam panel You can select the Office Diagnostics button to open the Office Services template to view or enter the various diagnostic tests and results. You can configure the physical exam systems in the Framework Content system template. Based on the configuration, the systems appear on Physical Exam of SOAP. Document a Multi System Exam on the SOAP Template

You can document the complete physical examination details of a patient on the SOAP template. The One Page Exam link in the left pane on the Physical Exam panel allows you to open the One Page - Physical Exam template. Note: Based on how the One Page Exam template is configured in the Physical Exam section on the Framework Content system template, and whether the Enable Gender Neutral PE checkbox is turned on in the SOGI/Transgender section on the Practice Configuration template, a gender-specific or a gender-neutral One Page - Physical Exam template opens. Document a Single System Exam on the SOAP Template

You can document the details of the single system examination of a patient in the Physical Exam panel on the SOAP template. Open NextGen® Enterprise EHR. Open the SOAP template. In the Physical Exam panel, select a single system exam in the left pane. Note: The left pane in Physical Exam displays the practice's custom list of the upper 14 single-system exams configured on the Framework Content system template. For more information on customizing the list, go to NextGen Healthcare Success Community, and download the Framework Content System Template for NextGen® Adaptive Content Engine. Select Additional in the list to select the required system from the complete list of systems that appears. The system-specific Physical Exam template opens. Enter the physical exam findings. Document a Physical Exam

The One Page Exam template is used to document a physical exam. Physical Exam panel You can examine a patient and document the information in the Physical Exam panel on the SOAP template. In the Physical Exam panel, you can do the follwing: Use the One Page Exam to document the exam on the Gyn-PE template. On the Gyn-PE select the Normal checkbox to indicate normal findings for each system or click the Findings field to add any abnormal findings. Use other single system exam and its sub menu in the left pane to document the corresponding exam details. When you click the single system exam or its sub menu, the respective template opens. Use the Office Diagnostics button to document or review diagnostics and to place orders for the tests. Use the Quick Load or Quick Save button to load a saved default set or save a new default set. Document Physical Exams

As part of the provider workflow, you can document and manage physical exams performed for a patient. Open NextGen® Enterprise EHR. Open the SOAP template, and then expand the Physical Exam panel. To document findings for multiple physical exams on a single template, select One Page Exam from the left navigation bar. Note: You can also select Load to quickly load any saved physical exam findings, or Save to save the documented physical exam findings to use them on other encounters. To document findings for a single physical exam, select an appropriate link from the left navigation bar. To document findings for a physical exam that does not appear in the left navigation bar, select Additional. To copy physical exam findings from a previous encounter to the current encounter, select Physical Exam History.If you have copied and edited information from a previous encounter, a label appears in the Physical Exam panel and applicable documents indicating that information is copied and edited.

Document a Physical Exam

The Physical Exam panel includes a list of exam categories in the left pane. The categories in the left pane are customizable by specialty or by provider on the Framework Content system template. For more information on the Framework Content system template, go to NextGen Healthcare Success Community, and download the Framework Content System Template for NextGen® Adaptive Content Engine. When you select the Physical Exam History button, the Historical Information window opens. You can copy the previous physical exam records to the current encounter by using the Copy Physical Exam & Close button. Select the One Page Exam link from the left pane.The applicable exam template opens. Note: The resulting window is dependent on the specialty selected. Enter any pertinent exam details. Select Quick Save to save the documented findings as the default set for the next subsequent encounters.
