Document a Breast Palpation Physical Exam

You can document the breast palpation details in the Physical Exam section on the SOAP template.

1. Open NextGen® Enterprise EHR.
2. Open the SOAP template.
3. In Physical Exam, select One Page Exam.
   The GYN-PE template opens.

   

4. In the Breast section, under Palpation, select the Details link.
   The Breast Palpation-PE template opens.

   

5. Enter the breast palpation details in the corresponding breast, that is, right, left, or bilateral.
   1. Enter the breast palpation and location details.
   2. Enter the distance from the areola in centimeter.
   3. Enter the size in centimeter.
6. Select Save & Close.
