Sign Off on Challenge Tests

You need to sign off the encounter after the completion of Challenge Tests. Open NextGen® Enterprise EHR. On the Testing - Allergy template, expand the Challenge Tests panel. Enter additional information in the Comments field. Select the Physician sign off checkbox.NextGen® Enterprise EHR fills the associated field with the current user's information and the Date and Time fields with the current date and time. Challenge Testing Evaluation

Following the completion of the Challenge testing, the provider must review the test results, evaluate the patient, and sign-off on the results. The patient is then discharged with instructions, if needed. Sign Off on Challenge Tests You need to sign off the encounter after the completion of Challenge Tests. Record Challenge Test Results

You can record a challenge test result on the Testing - Allergy template. Open NextGen® Enterprise EHR. On the Testing - Allergy template, expand the Challenge Tests panel. To enter the results, select the desired entry in the grid. NextGen® Enterprise EHR fills the fields above the grid with the data from the selected entry. In the Results section, do the following: Enter the time of the interpretation of the test. Enter the patient's vitals details. Select Reaction to select the reaction that best matches the patient's current state. Select the Interpretation field and select Positive or Negative from the list. Enter additional details in the Comments field. Select Update. NextGen® Enterprise EHR adds the data to the selected entry in the grid. Record Vital Signs for the Challenge Test

You can document the vital signs of a patient to perform the Challenge Tests. Open NextGen® Enterprise EHR. Open the Testing - Allergy template and expand the Challenge Tests panel. To select the type of challenge test from the list, in Administration, select Type.A list appears showing the allergens that are ordered. Select the allergen.NextGen® Enterprise EHR fills the Type and Allergen fields automatically. To enter the time, do one of the following: Select the stopwatch icon for the Time field under Results to record the current time. Select the drop-down arrow for the Time field and use the Time Entry window to enter the time. Record the patient's vital signs details. To indicate that the recorded vital signs are the initial set, select the Baseline checkbox. Select Add. NextGen® Enterprise EHR adds the baseline vital signs to the grid and inserts a dashed line below the entry. The baseline entry remains at the top of the grid. Document Prick, ID, and Challenge Test Results

You can document interpretations for Prick, ID, and Challenge allergy tests in the Results section of the Prick/ID/Challenge panel. Open NextGen® Enterprise EHR. Open the Testing - Allergy template. In the Results section of the Prick/ID/Challenge Tests panel, select the appropriate Test type. The ordered allergens appear in the Results grid. Note: The system displays the first group of 20 allergens. To view each subsequent group of tests, select a group link to view the corresponding group. CAUTION: If you enter any data for a current group of allergens, you must save that data before displaying the next group of tests. NextGen® Enterprise EHR clears the fields when you select another group of tests. Enter the test results for each allergen. To indicate negative results for all allergens in the grid, select All negative. Do one of the following to record the time of Interpretation: Select the stopwatch icon for the desired field. Perform a Skin, Oral, or Medication Challenge Test

You can perform a skin, oral, or medication challenge test for a patient in the Challenge Tests panel. Open NextGen® Enterprise EHR. On the Testing - Allergy template, expand the Challenge Tests panel. After entering baseline vital signs for the challenge test, administer the first dose of the test. To document the allergen administration time, do one of the following: To record the current time, select the stopwatch icon for the Time field under Administration. To enter a different time, select the drop-down arrow for the Time field and use the Time Entry window to enter the time. Select the drop-down arrow for the Amount field to enter the amount of allergen administered. Select the blank field next to the Amount field and select the measure of the amount administered. Select Add. Note: Each entry appears at the top of the grid, below the baseline values, for easy comparison to the baseline values. Parent topic: Challenge Tests Panel
