Care Transitions Panels

Panel	Description
General	Use this panel to enter a patient’s general care transition information, including the primary care physician, case manager, and the facility admitted to or discharged from.
Discharge Diagnosis	Enables you to enter a diagnosis for a patient that can be used for continuity of care if the patient is being transitioned to another provider. The Discharge Diagnosis panel documents the diagnoses information after the patient is discharged from the hospital/facility.
Problem List after Discharge	The Problem List after Discharge panel allows an update of the patient’s problem list, based on the discharge information received.

Update a Patient's Problem List

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. On the Care Transitions tab, and expand the Problem List after Discharge panel. Select the appropriate checkbox to build the description of services required for the patient. If required, type additional text into the description. If the patient currently does not have any active problems, select the No active problems option. To add a new problem to the list, select Add.The Problem module appears. Note: For information on using the Problem module, go to the NextGen Healthcare Success Community, and download the Problems Module User Guide for NextGen® Enterprise EHR. Record a Discharge Diagnosis

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. Select the Care Transitions tab.The Cm Care Transitions template opens. Cm Care Transitions On the General panel, select a blank Discharge diagnosis field.The Diagnosis Search window opens. Use the window to locate and select the appropriate diagnosis.The selected record appears in the Discharge Diagnosis field in the Discharge Diagnosis panel. Note: You can add up to four diagnoses. Select the appropriate option for the Previous hospital admission in last 30 days field. Select the appropriate option for the Complete D/C summary received field. Parent topic: Care Transitions Tab

Document General Transition of Care Information for a Patient

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. Select the Care Transitions tab.The Cm Care Transitions template opens. Cm Care Transitions Select the PCP field.The field populates with the Primary Care Practitioner established in the patient's demographics. If there is no PCP selected in the patient chart, the Modify Patient Information template opens, enabling you to select a PCP. Do one of the following: Select the Encounter Type field to launch a list of transitional care encounter types and then select an encounter type from that list. Select the blue arrow to the right of the Encounter type field to launch the interim history data list and then select a past encounter from the interim history data. Comp Assessment Problem List Panel

The Problem List panel displays a list of patient's chronic and secondary problems documented in the Problem List after Discharge panel on the Care Transition template. You can make changes in any of the documented problems by highlighting a problem in the grid and clicking the Edit button. The Add button enables provider to add any new problem. Document Transitioning into Care from Any Specialty

You can document transitioning into care from any specialty affected by transition into Care Management. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Select a Specialty that is affected by Transitioning into Care Management and select Visit Type as Transition into Care. In the General Information panel, select the Transitioning Into Care link.The Transitioning Into Care template opens. Transitioning Into Care On the Transition into Care Management panel, select the Discharged field, and select the discharged date from the calendar. Select the Initial Communication field, and select the appropriate option from the list of communication types. Select the on field, and select the date of the initial communication from the calendar.
