Edit OPTIMAL Instrument
window opens.
Add difficulty and confidence scores for all activities.
In the
Prioritization
section, define three priorities.
The activity group for the first priority appears in the
ICF Activity
field.
Click
Save & Close
.

Edit Activity Scores and Priorities
You can only edit the scores and priorities of today's encounter. Open NextGen® Enterprise EHR. Open the OPTIMAL Instrument template. Click Edit.The Edit OPTIMAL Instrument window opens. Edit difficulty and confidence scores for activities. In the Prioritization section, define three priorities. You must enter the three priorities every time you open the Edit OPTIMAL Instrument window.The activity group for the first priority appears in the ICF Activity field. Click Save & Close. View Activity Scores and Priorities
You can view a patient's difficulty and confidence scores for activities in the following groups on the OPTIMAL Instrument template: Changing & Maintaining Body Positions Walking & Moving Carrying Moving & Handling Objects Open NextGen® Enterprise EHR. From the Physical Therapy template, click the OPTIMAL link.The OPTIMAL Instrument template opens. The group that the first priority belongs to appears in the Currently reporting on field. Optimal Instrument To view another group in the Currently reporting on field, edit the priority on the Edit OPTIMAL Instrument window. To view difficulty and confidence scores from today's visit, select Today's visit. To view difficulty and confidence scores from all visits, select All current visits. To view activities in the group that appears in the Currently reporting on field, select the Filter by related reporting activity checkbox. Add a Patient Goal to Address a Health Concern
You can add multiple goals to a single health concern. To add multiple goals, repeat this procedure for each goal. Open NextGen® Enterprise EHR. Expand the Goals, Interventions and Outcomes panel. The Select a Health concern to document section appears with all previously added health concerns. Goals, Interventions and Outcomes panel Select a health concern from the list. The information about the selected health concern goal fields appears on the right. Goal details on the Goals, Interventions and Outcomes panel In the Goal field, type a goal description to address the health concern, or use Common Phrases. Indicate if the goal is a patient goal or a provider goal. Verify that the life cycle status of the goal is set to Active. You can also change the status. Add an Action Item
Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. Open NextGen® Enterprise EHR. On the Tracking template, expand the Action Item List panel, and select Add. Select Add.The Care Management Tasks template opens. New Record Window The Time, Date and Staff fields are populated with current information. Select the Due date field and select a date from the calendar. Select the Priority field and select a priority from the list. Type any relevant information about the task in the Details field. Select the Status field and select the status from the list. Create an Intervention to Achieve a Goal
You can add multiple interventions to a single goal. To add multiple interventions, repeat this procedure for each intervention to be added. Open NextGen® Enterprise EHR. Open to the Goals, Interventions and Outcomes panel.The Select a Health concern to document grid is populated with all previously identified health concerns. Select a health concern from the list.The selected health concern appears in the Health Concern field and the data entry fields for goals appear to the right. Select the goal that you are adding an intervention for.The selected goal appears in the Goal field and data for the selected goal fills the data entry fields to the right. Create a Task for an Intervention
Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. If an intervention requires an action by a provider, you can create a task for that provider. The tasks you create are not linked to a particular intervention and behave as any other task in the tasking function. Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Interventions panel. Select the intervention for which you need to create a task and select Task.The To Do window appears. To Do The Date Due and Priority fields are pre-filled with current date and priority. Note: If you want to specify a different date or priority, select the corresponding fields and then select appropriate value. Select a Subject and a Category. Type the message in the Message field.
