Document Information about Individuals Present during an Assessment

You can document details about the individuals present during the patient's assessment. On the assessment template, expand the Individuals Present panel. Select the contact from Contact Type. Information in POS Description and Code is filled in automatically. Select the reason for contact. Indicate whether the individual is present and, if not, enter the reason for the absence. If other individuals are present, select Others present, and then enter information about those individuals. To enter information about a support resource person, select Add. In the Guardian Info window, do the following: In the Patient Relationship/Support Role section, enter information about the relationship with the foster child. Enter the support person's role, name, and dates for the support period. In the Residence section, enter the primary and secondary addresses. Enter details about the primary and secondary communication and the representation timeframe. Select Save. The relationship details are saved

Document Goals, Objectives, and Interventions

You can document the patient's goals, objectives, and interventions. On the assessment template, expand the Goals, Objectives, and Interventions Addressed Today panel. Goals, Objectives, and Interventions Addressed Today To view goals linked to the current encounter’s program, select the Program filter option. Note: If the filter is not available, your practice has not turned on Programs to be linked to Goals/Objectives/Interventions on the BH Foundation Configuration template. For more information on configuring programs to link to goals, objectives, and interventions, go to NextGen Healthcare Success Community, and download the Configuration Guide for NextGen® Behavioral Health Suite. Select the current goal from Goal today.The Objective today and Intervention today fields are automatically filled with the information in the Integrated Service Panel (BH) template. Document Group Visit Information

You can document the details of group sessions, including the therapy progress of each individual. There are several types of psychotherapy visits, of these, group visits are a common occurrence in the behavioral health setting. Family and group psychotherapy visits can only be completed for a patient who is already designated as a member of a group in the system. Open NextGen® Enterprise EHR, and then select File > New > Group Encounter. Choose a patient group, and then select OK. To access the Group Prog Note (BH) template, select Select Template Template Icon The Select Template window opens. On the Group tab, select All. Select Group Prog Note (BH). Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list. You can select <No Program> to create an encounter without a program, and then add the program later. Document the Patient's Intake Notes Using the Comprehensive Assessment Templates

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. From the list, select Comp Assess - Concerns, and then select OK. Select the program, specialty, and visit type for the patient. Complete the assessment in the Concerns tab. Tasking Supervisors and Staffs for Review and Signoff

You can use the Signature (Tasking Supervisor/Staff for review and signoff) panel to request secondary approval or attestation from another user. You can send a task to the approver requesting approval. The approver can use this panel to add their electronic signature and generate a Signature Attestation document that is separate from the template document. By having a separate document for attestation, staff can review and attest even after the client has signed the original template document with a signature pad. Note: Only use the Signature (Tasking Supervisor/Staff for review and signoff) panel when you need secondary approval. When secondary approval is not needed, skip this panel as the system electronically signs the document for the provider completing the template.
