Document Pharmacy Details

You can document up to two pharmacies details of the participant. Open NextGen® Enterprise EHR. Open the CW Demographics (BH) template. Expand the PCP/Primary Practice Provider/ Pharmacy panel. Select Pharmacy.The Modify Patient Information window opens. Select Pharmacy from the left pane. Modify Patient Information Window Panel In the Pharmacy Info section, select the folder icon, and then select New.The Pharmacy Lookup window opens. To add pharmacy details, do one of the following: To link pharmacy details already in the system, search for the pharmacy, select it, and then select OK. To enter new pharmacy details, enter details of the pharmacy, and then select OK. Select OK. Note: You can add up to two pharmacy details. The pharmacy details are updated in the panel. Referral Inquiries and Histories

This feature allows you to submit, or request, new referral inquiries and view referral histories. Referral requests are made to generate a referral number so that a primary care physician (PCP) can send a patient to an authorized specialist. Referral history enables you to view a patient's history. This history lists all previous referrals made for a patient. Referral history generally documents the history of a patient treated by a specialist. Clinical Pharmacist Workflow

Clinical pharmacists can review the patient details and provide medication recommendations using the Clinical Pharmacist Recommendation template. During a patient's encounter, a provider can involve a licensed clinical pharmacist to provide medication recommendations. The clinical pharmacist can be at provider's office or at the same practice. For the Family Practice, Internal Medicine, and Pediatrics specialties, the clinical pharmacist uses the Clinical Pharmacist visit type to review the patient details and provide medication recommendations using the Clinical Pharmacist Recommendation template. Open the Person Information or Patient Information window for the desired person or patient. Select the Provider tab. Select the row for the provider to be modified. Select the open menu , and select Open. The Modify Patient Provider window appears. Modify the following fields as needed: Preference Name Provider Effective/Expiration Dates Expiration Reason Primary Preference Name To save changes and close the window, select OK.
