Document Interventions to Lower the Risk Level
Open the SAFE-T Protocol with C-SSRS template and expand the Guidelines to Determine Level of Risk and Develop Interventions to Lower Risk Level panel. Guideline to Determine Risk Level and Triage Plans You can override the Risk Stratification category based on your clinical judgment. Note: The risk stratification category is defaulted to the answers documented for questions 1 to 6 in the Identify Risk Factors panel. Select the triage plans against each of the risk stratification categories that can lower the risk level. Select any clinical notes and add your comments. Open the SAFE-T Protocol with C-SSRS template and expand the Specific questioning about Thoughts, Plans, and Suicidal Intent panel. Specific Questioning on Thoughts, Plans, and Suicidal Intent Document the answers to all of the questions. Note: The total score is calculated only if you document answers for all of the questions. SAFE-T Protocols with C-SSRS
The Columbia Suicide Severity Rating Scale, or C-SSRS, is a suicidal ideation and behavior rating scale that rates an individual's ideation on a scale, ranging from "wish to be dead" to "active suicidal ideation with specific plan and intent and behaviors". Questions are phrased for use in an interview format. But, the C-SSRS assessment questions alone do not meet some regulatory requirements. Another screening tool is introduced that includes the SAFE-T protocol questions along with the C-SSRS questions. As a provider, you can document the suicidal plan and attempt of an individual to assess the risk level. Document Risk Assessment
Open a risk assessment template, and expand the Risk Assessment panel. To document a potential for suicide and homicide risk, select one or more of the following links: Suicidal/Homicidal Risk Safety Risk Columbia Suicidal Severity Rating Scale (C-SSRS) SAFE-T Protocol with C-SSRS (Columbia Risk and Protective Factors) Note: Only one of the C-SSRS template appears based on the configuration in the BH Foundation Configuration template. To open the risk history view, select the Risk Assessment History link.The Risk Assessment History window opens. If the client is not currently and has never been a danger to themselves, others, or property, select the respective checkbox. If selected, the remaining fields are turned off.
