Document the Patient's Referrals for Psychiatric Rehabilitation

You can document referrals, treatment recommendations, assessed needs, and discharge planning for psychiatric rehabilitation patients. You can document the directed care plan and services that are provided as part of the psychiatric rehabilitation services.

1. Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates.
   The Select Template window opens.
2. On the Medical Records tab, select All.
3. Select Rehab Referral (BH), and then select OK.
   Note: You can open the Rehab Referral (BH) template from your Packet Navigation,or from the Templates module. For more information on configuring Packet Navigation, go to NextGen Healthcare Success Community, and download the Configuration Guide for NextGen® Behavioral Health Suite.
4. Select the program, specialty, and visit type.
   Note: Only programs that the patient is actively enrolled in appear in the list. You can select <No Program> to create an encounter without a program, and then add the program later.
5. Document the general details of the referral.
6. Document the provider details.
7. Document the patient's primary care provider's details about the shared information.
8. Document the reasons for the referral.
9. Document the requested services, such as self-care skills, social skills, and independent skills.
10. Document the symptoms and risks of the referral.
11. Document the assessment and active diagnosis details.
12. Document the medications of the referrals.
13. Document the hospitalization details.
14. Select whether the referral currently receives any mental health services.
15. Select Generate Document.
