Document the Patient's Medical and Surgical Details

  1. Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates.
    The Select Template window opens.
  2. On the Medical Records tab, select All.
  3. From the list, select Comp Assess - BH Hx, and then select OK.
  4. In the Past Medical/Surgical panel, select Add to document the patient's medical and surgical details.
  5. To add an item to the Medical or Surgical panel, select the respective checkbox for the condition or surgery.
    The Onset Date (in the Medical panel) or Date (in the Surgical panel) field and the Details Details Icon icon appear beside the selected item.
  6. To add problems that are not available on the Past Medical History template, use the Other field.
    • Use the Onset Date field in the Medical panel to enter the date when the condition first appeared.
    • Use the Date field in Surgical panel to enter the date the surgery was performed.
  7. Select Onset Date or Date to access the Date Selection window.
    The Data Selection window opens.
  8. You can enter the Onset date manually into Onset date or by selecting the calendar to select a date.
  9. Enter the approximate patient age in Onset age.
    Onset age can be entered as days, weeks, months, or years but a future onset age cannot be entered.
  10. Select Save & Close.
  11. To add an additional description for the condition or surgical procedure, select the Details, located beside the selected condition or procedure.
    The Manage Past Medical/Surgical History template opens.
  12. Select Save to Grid & Close to return to the Past Medical History template.
    The details of the selected condition or surgery appear in the grid in the Past Medical History Grid panel.
  13. To clear a condition, surgery details, or entries in the Past Medical History grid, select a row in the grid and select Remove.
  14. Select OK.
