Document General Addiction Severity Index Information

From the Addiction Severity Index (BH) template, select a Program, and then select OK.
Expand the General Information panel.

The Person number, Medical record number, Other ID number and Gender fields in the General panel are read only.
To view or add the client's social security number, select the Social Security Number link to launch the Modify Patient Information window.
In Date of admission and Date of interview, select the corresponding dates from the calendar.
In Time begun, enter a time in the field or select Start.
By selecting Start, the Time begun and Date of interview fields update with the current date and time.
In Time ended, enter a time.
For Class, select Intake or Follow-up.
For Contact Code, select In person, Telephone, or Mail.

Note: The Intake ASI must be completed in person.
By default, the Interviewer field displays the current user. To change the name, select in the field and make the updates.
In Special code, select in the field and select an option from the picklist.
To document additional general addiction severity information, select the Continue General Information link.

Document Additional General Addiction Severity Index Information

The Addiction Severity Index - General window enables you to document general demographic information for the client. You can select for more information on completing the documentation on this window. On the Addiction Severity Index - General template, the Name, Address, and Date of birth are read only fields. To open the Addiction Severity Index - General window, do one of the following: From the General Information panel, select the Continue General Information link. From the Severity Profile panel, select any of the links listed in the panel and then go to the General tab. To display helpful information regarding each question, select the question number. Severity Profile

The Severity Profile panel enables you to document the addiction severity index in key areas such as medical, employment/support, alcohol/drug, legal, family/social, and psychiatric. You can access each of the areas by selecting on the corresponding links in the Severity Profile panel. By selecting on a link, a corresponding template opens with tabs with the associated tabs across the top. You can document all areas by selecting Next. Managing Patient's Addiction Details

Your practice can configure the Addiction Severity Index (BH) to be accessible from your Packet Navigation or you can access the template from the Templates module . For more information, go to NextGen Healthcare Success Community and download the Configuration Guide for NextGen® Behavioral Health Suite. On the General Information and Severity Profile panels, you can select the information icon for tips on using the specific panels during the interview. After documenting the potential problem areas for the client, you can submit charges for the encounter or select the No Billing for Service checkbox. The Addiction Severity Index - Medical window enables you to document the Addiction Severity Index (ASI) medical information for the client. You can access the Addiction Severity Index - Medical window by selecting on the Medical link on the Severity Profile panel on the Addiction Severity Index (BH) template. You can access the ASI psychiatric questions by selecting on the Psychiatric link on the Severity Profile panel on the Addiction Severity Index (BH) template.
