Document Care Coordination

  1. Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates.
    The Select Template window opens.
  2. On the Medical Records tab, select All.
  3. Select Care Coordination (BH), and then select OK.
  4. In the Time Entry Navigation sidebar, select Billing Time Entry, and then enter the start time.
  5. Select the program, specialty, and visit type.
  6. In the Care Coordination Team panel, document agency and interdisciplinary team details.
  7. In the Barriers to Care panel, document the potential barriers to care, and then record work done to resolve those barriers.
  8. In the Care Plan History panel, document the care plans for the patient such as needs, goals, outcomes, interventions and progress details.
  9. In the Care Coordination panel, document the caregiver details.
  10. In the Education/Recommendations panel, document additional information and recommendations for the patient.
  11. In the Action Items panel, document the future courses and follow-up details for the patient.
  12. Document the patient's assessment and diagnosis details.
  13. Document the applicable signatures from the tasking supervisor or staff.
  14. In the Time Entry Navigation sidebar, select Billing Time Entry, and then enter the end time.
  15. In the Charges panel, document the billing details.
  16. Optional: Select No Billing for Service if there is no charge for the current date.
  17. Select Generate Document.
