The Addiction Severity Index - Alcohol/Drug windows enable you to document the Addiction Severity Index (ASI) alcohol and drug use for the client. You can access the Addiction Severity Index - Alcohol/Drug questions by selecting on either the Alcohol or the Drug link on the Severity Profile panel on the Addiction Severity Index (BH) template.
- Complete the questions on the Addiction Severity Index - Alcohol/Drug 1 window.
Use the Comments field to enter additional information, if applicable.
- To display helpful information regarding each question, select the question number.
Information regarding what information to provide for the question displays on the template.
- To clear a field, select X.
- To move to the next set of questions, select Next or select a corresponding Items link at the bottom of the template.
- On the Addiction Severity Index - Alcohol/Drug 2 and Addiction Severity Index - Alcohol/Drug 3 windows, repeat steps 1-4.
- For additional comments, use the Alcohol/drug section comments field.
- Do one of the following:
- To save and move to the previous set of questions on the Addiction Severity Index window, select Previous.
- To save and move to the next set of questions on the Addiction Severity Index window, select Next.
- To save and return to the Severity Profile panel on the Addiction Severity Index (BH) template, select Save & Close.
- To close without saving, select Cancel.
After you select Save & Close, the information updates and displays in the corresponding column of the grid in the Severity Profile panel.
