Add Patient's Need, Goal, Objective, and Intervention Information

  1. In NextGen® Enterprise EHR, in Patient History, select the Templates button.
    The Select Template window opens.
  2. On the Medical Records tab, select All.
  3. From the list, select Integrated Service Plan (BH), and then select OK.
  4. Expand the Assessed Needs, Goals, Objectives, and Interventions panel.
  5. Select the assessed Need record to display the goals, objectives, and interventions.
  6. To add a Need, select Add next to the Assessed Needs grid.
    The Assessed Need Information window opens.
  7. Select the details, and then select Save.
    The record appears in the Assessed Needs grid and the Tis Dbp Bh Assessed Needs list.
  8. To resolve a Need, select the need in the grid, and then select Resolve.
    A confirmation message appears.
  9. Select the Linked to assessed need field.
    The Tis Dbp Bh Assessed Needs list opens, which is a list of all active needs identified in the Assessed Needs grid.
  10. Select a need from the picklist, and then select OK.
    The assessed need appears in the Linked to assessed need field.
  11. For Goal status, select the appropriate option.
    If the Goal status is Attained or Discontinued, enter the attained or discontinued date in the respective field.
  12. Enter the information in each box provided for the following fields:
    • Goal - This field is mandatory. The ISP Goal DB picklist may be program-specific. Qualifier 1 on the BH Content panel of the BH Foundation Configuration template is used to filter the list.
    • Objective - This field is mandatory. The ISP Objectives DB picklist is program specific. Qualifier 1 on the BH Content panel of the BH Foundation Configuration template is used to filter the list.
    • Intervention - This field is mandatory. The ISP Intervention DB picklist is program-specific. Qualifier 1 on the BH Content panel of the BH Foundation Configuration template is used to filter the list.
    • For all other fields, enter the details, as applicable.
  13. Enter the date of expected accomplishment.
  14. Select Add.
    If you select Add without completing the required Goal, Objective, or Intervention field, the record does not get saved.
    The goal appears in the Goal grid. A goal number is assigned to each goal, objective, or intervention combination when it is created. This goal number acts as a unique identifier for this combination in grids and picklists and displays the combination in chronological order.
  15. To display only active goals on the Integrated Service Plan document, select the Include only active goals on document checkbox.
    Only goals with a status New, Continued, or Revised appear in the document. Goals that are Attained or Discontinued on or after today's encounter also appear on the document.
  16. To sort the grid in the document based on the goal number, select Ascending or Descending.
