Add Patient's Need, Goal, Objective, and Intervention Information
- In NextGen® Enterprise EHR, in Patient History, select the Templates button.
The Select Template window opens.
- On the Medical Records tab, select All.
- From the list, select Integrated Service Plan (BH), and then select OK.
- Expand the Assessed Needs, Goals, Objectives, and Interventions panel.
- Select the assessed Need record to display the goals, objectives, and interventions.
- To add a Need, select Add next to the Assessed Needs grid.
The Assessed Need Information window opens.
- Select the details, and then select Save.
The record appears in the Assessed Needs grid and the Tis Dbp Bh Assessed Needs list.
- To resolve a Need, select the need in the grid, and then select Resolve.
A confirmation message appears.Note: If you resolve a Need, the associated goals, objectives, and interventions are discontinued. If you update the Need to a status other than Resolved, the associated goals, objectives, and interventions remain discontinued.
- Select the Linked to assessed need field.
The Tis Dbp Bh Assessed Needs list opens, which is a list of all active needs identified in the Assessed Needs grid.
- Select a need from the picklist, and then select OK.
The assessed need appears in the Linked to assessed need field.
- For Goal status, select the appropriate option.
If the Goal status is Attained or Discontinued, enter the attained or discontinued date in the respective field.
- Enter the information in each box provided for the following fields:
Note: If your practice has configured the Enable Pre-Configured Goals, Objectives and Interventions Picklist option on the BH Foundation Configuration template, a corresponding picklist launches when you select the corresponding Goal, Objective, or Intervention fields.
- Goal - This field is mandatory. The ISP Goal DB picklist may be program-specific. Qualifier 1 on the BH Content panel of the BH Foundation Configuration template is used to filter the list.
- Objective - This field is mandatory. The ISP Objectives DB picklist is program specific. Qualifier 1 on the BH Content panel of the BH Foundation Configuration template is used to filter the list.
- Intervention - This field is mandatory. The ISP Intervention DB picklist is program-specific. Qualifier 1 on the BH Content panel of the BH Foundation Configuration template is used to filter the list.
- For all other fields, enter the details, as applicable.
- Enter the date of expected accomplishment.
- Select Add.
If you select Add without completing the required Goal, Objective, or Intervention field, the record does not get saved.Note: If your practice has enabled the Enable Programs to be linked to Goals/Objectives/Interventions configuration option on the BH Foundation Configuration template, the goal is linked to the encounter’s Program selection.The goal appears in the Goal grid. A goal number is assigned to each goal, objective, or intervention combination when it is created. This goal number acts as a unique identifier for this combination in grids and picklists and displays the combination in chronological order.
- To display only active goals on the Integrated Service Plan document, select the Include only active goals on document checkbox.
Only goals with a status New, Continued, or Revised appear in the document. Goals that are Attained or Discontinued on or after today's encounter also appear on the document.Note: The following list represents the grid filters:
- Need field filters:
- All: the default filter that displays all Need records that have an associated Goal.
- Goal field filters:
- All: the default filter that displays all Goal records.
- When you select a goal as filter, the grid is sorted by the Objective column.
- Program field filters:
- All: the default filter that displays all Program records.
- Updated goals are associated with the current encounter and the same appears as a filter in the list.
- Goal Status field filters:
- Active: the default filter that displays the records with the following status values:
- New
- Continued
- Revised
- Attained: displays all goals with an attained date.
- Discontinued: displays all goals with a discontinued date.
- Active: the default filter that displays the records with the following status values:
- Documented By field filters:
- All: the default filter that displays all Documented By records.
- Whenever a goal is updated, it is associated with the current encounter and the same appears as a filter in the list.
- Select Clear All to reset all filters to their default values.
- Need field filters:
- To sort the grid in the document based on the goal number, select Ascending or Descending.
Note: By default, Ascending is selected based on the configuration on the BH Foundation Configuration template. The first sorting of the generated grid is based on the Program followed by the goal number.
