Add Family Information
Open the
Patient Information
window for the desired patient.
Select the
UDS
tab.
In the
Family Information
section, select the
Open Menu
button, and select
New
.
Note:
A head of household must be selected for a patient to enter family information.
The
Family Info
window appears.
In the
Family Size
field, enter the number of family members in the patient's household. The default value is 1.
In the
Income
field, enter the income amount for the patient's household. The default value is $0.00.
In the
Income Cycle
field, select one of the following. This field is required.
Annually
Monthly
Weekly
If the head of household refuses to provide income information, select the
Refused to Report
check box. The
Income
field becomes unavailable.
In the
Verify Date
field, enter the date that the family information was verified. This field is required.
In the
Effective Date
field, enter the start date for the family information. This field is required.
In the
Expiration Date
field, enter the end date for the family information. This field is required.
Complete the
Sliding Fee Schedule Verification
section.
To save changes and close the window, select
OK
.

Add a Sliding Fee Schedule
Open the Person Information or Patient Information window for the desired person or patient. Select the UDS tab. In the Sliding Fee Schedule Verification section, select the Open Menu button, and select New. The Sliding Fee Verification window appears. In the Sliding Fee Schedule field, select a sliding fee schedule for the patient. This field is required. Note: Items available for selection are from the Sliding Fee Schedules master file in File Maintenance. Hidden and expired sliding fee schedules are not listed. In the Effective Date field, enter the start date for the sliding fee schedule. This field is required. In the Expiration Date field, enter the end date for the sliding fee schedule. This field is required. If a head of household has been entered for the patient, complete the Head of Household Family Information section. Modify Family Information
Open the Patient Information window for the desired patient. Select the UDS tab. In the Family Information section, select the Open Menu button, and select Open. The Family Info window appears. Modify the following fields as needed. Family Size (see note) Income (see note) Income Cycle (see note) Refused to Report (see note) Verify Date Effective/Expiration Dates To save changes and close the window, select OK. Note: Family information can be modified only if it has not been used. If family size and income were previously used to calculate automatic adjustments on an encounter, they cannot be modified. Family Information
In the Family Information section of the UDS tab, you can enter family size and annual income for the patient's head of household. This information is typically used by Community Health Centers in conjunction with sliding fee schedules. When a patient qualifies for a sliding fee schedule, family size and income are used to determine the percentage of charges that are automatically adjusted for the patient. Family information is also reported on the UDS Report. Note: A head of household must be selected for a patient to enter family information. Family Information is stored at the practice level; therefore, each practice can enter and maintain different information for a person or patient.
