The Claim Printing window displays the claim print options based on the paper claims and the payer type you select.
The following paper claims appear in the Claim Printing library:
- 1500 Claims enables you to select CMS 1500 (2005) or CMS 1500 (2012) claim form. The options for the form you select appear under 1500 in the Option column.
- UB Claims enables you to select UB04 (2004) or UB92 (1992) claim form. The options for the form you select appear under UB in the Option column.
- ADA Claims enables you to select ADA (2019) or ADA (2024) claim form. The options for the form you select appear under ADA in the Option column. ADA (2024) is the default claim form when you set up a new Claim Printing library.
The following types of payers appear in the Claim Printing library:

| Tertiary | Enables you to set the claim print options that apply only to the tertiary payers. |
